- On Thursday, the U.S. Navy released results of four investigations into incidents involving the USS Harry S. Truman from Dec. 22, 2024 to May 2025, costing over $100 million and causing three lost aircraft and carrier damage.
- Investigators found mishaps linked to high operational tempo, fatigue, inadequate training between USS Gettysburg and the Truman strike group, and maintenance shortfalls causing arresting gear and trip wire failures.
- The Gettysburg watch team listed two aircraft as `unknown` despite friendly data, interviews said the Identification Friend or Foe system was `a bit finicky`, and during the 40-minute post-incident timeline, conflicting commands caused confusion while one F/A-18F crew ejected and was recovered.
- Vice Chief of Naval Operations Adm. Jim Kilby said accountability actions were taken across operators, Truman's commander Dave Snowden was relieved in February, and repairs plus personnel training are planned at Newport News Shipbuilding.
- Martin said `The clear message from this deployment is that the Navy is not ready to deal with the reality of extended combat`, and investigators urged reforms including training and maintenance after the USS Harry S. Truman's eight-month deployment.
