Bringing Healthcare Closer to Home: How to Reach Rural Patients
The program uses telehealth and remote monitoring to help chronic care patients stay connected while reducing long trips and easing strain on rural teams.
On Tuesday, Primefocus Health and the Marshall Health Network launched a connected care program serving patients across West Virginia, southern Ohio, and eastern Kentucky to improve chronic condition management.
With 80% of rural America medically underserved, rural residents often face hours-long travel for routine appointments, leading to poorer health outcomes from chronic conditions like diabetes and heart disease.
The digital platform enables remote monitoring and insights to guide recovery from home, while avoiding replacement of existing healthcare workers to ensure natural workflow integration.
Primefocus Health CEO Darren Sabo emphasized that tools must be "designed for the people who need it most," noting patients stayed engaged and chronic conditions improved under the program.
While digital innovations like AI and remote patient monitoring offer potential, technology alone cannot solve access challenges, as personal relationships with healthcare providers remain crucial for rural health outcomes.
