Major failings found after surgeon harmed women

A review of 325 cases found 48 women suffered major harm due to unconsented surgeries and poor clinical oversight by Daniel Hay, prompting NHS trust reforms.

  • On Wednesday, the Gynaecology Review Steering Group published a report on Daniel Hay, former obstetrician and gynaecologist, finding failings in care between 2015 and 2018 and identifying two women with severe and three with moderate physical harm.
  • The review found Mr Hay suffered deteriorating health, lacked insight, took clinical shortcuts including no pre-operation scans, and trust management delayed responses amid the Derby and Burton hospitals merger while theatre staff and consultants were only interviewed in 2022.
  • The panel contacted 325 women treated by Mr Hay between 2015 and 2018 and graded each case red, amber or green, with 48 rated red, 68 amber, and one woman offered a hysterectomy after a 10-minute consultation.
  • The trust apologised and said it was `truly sorry` to women affected, has improved consent and oversight, and Derbyshire Police continue a criminal investigation with Mr Hay interviewed earlier this year.
  • Reviewers recommended steps to identify poorly performing clinicians, prevent consultants working alone, and mandate multidisciplinary meetings; a phase 2 review will start in the autumn, while April 2020 patient letters created unjustified anxiety.
