The driver of a bus involved in a fatal crash in Virginia failed to slow for traffic ahead of a work zone, causing a pile-up that killed five people from Massachusetts, according to a preliminary report from government investigators.The National Transportation Safety Board released its report into the May 29 crash on Thursday. A family of four from Greenfield was killed, as well as a woman from Worcester.
